Community Support Specialist (Information Technology)



Reporting to the Program Manager, Community Support Services, the Community Support Specialist will provide case management, linkages, coordination, and support as needed by consumers. In addition, this position under the supervision of the Community Supports Program Manager will conduct initial assessments and reassessments, face-to-face home visits, and community visits at a minimum of once every 90 days, develop goals and a care plan with the consumer based on the consumers strengths and assessment data.
The position will demonstrate knowledge of health care, social services, employment, housing, recreational opportunities, and other services and systems available in the community. This role strives to help and empower individuals with mental health illnesses through community support. By maintaining current knowledge of available community resources and community outreach program options, this role actively participates in helping a vulnerable population reach resources to improve their daily lives.
