RN Program Coord-Heart Clinic (Healthcare)
Job Summary
Defines the general, level and purpose of the work performed by this job.
Provides nursing care across the continuum of care services for patients with structural heart disease that may require a broad
spectrum of advanced surgical or interventional treatment and services. This position may be involved in research clinical trials,
patient and professional educational programs and patient follow-up. Emphasis is on the team approach. The role of the Nurse
Coordinator will evolve over time to meet the needs of the patients and of the advanced structural heart disease cardiovascular
team.
Essential Accountabilities

• Performs nursing assessments on patients by collecting health data and determining specific health needs including
psychosocial evaluation; renders nursing judgments which adhere to accepted health practice; coordinates services and tests to
promote continuous comprehensive evaluation and care.
• Documents assessments, evaluations, recommendations, consultations and other pertinent information in the patient's medical
records and in clinic navigation data base.
• Coordinates the referral, screening, additional patient testing requirements, consult appointments and hospital-based
procedures. Facilitates communication with cardiovascular surgeons and cardiologists, including referring physicians regarding
patient eligibility and clinical status.
• Collaborates with the nursing staff and educators, case managers and outpatient services in an acute care environment to
ensure prompt movement of patients along the continuum of care. Provides professional education to clinical team members in
the advanced techniques and care of patients receiving specialized interventions.
• Facilitates the progression of the structural heart procedure patient and assists with preparing the patient and family. Provides
follow-up care per pre-determined clinic intervals, documents patient status and communicates ongoing patient condition to
physicians during follow-up care.
• Collaborates with an interdisciplinary cardiovascular health care team in planning, implementing and evaluating ongoing care
programs and education for the patients, families, care givers and community.

Job Description
• Oversees program development and ongoing maintenance for the program to meet ever-changing business climate. In
conjunction with physicians and program director, development may include but is not limited to: clinical care forms, patient flow
charts, patient and physician professional communication tools, clinical care protocols, patient data tracking methodology (using
4D) and program goals and outcome reporting.
• Provides community physician outreach and education using nursing experience and networking in conjunction with physician
leadership to build the referral program within Community Medical Centers' expanding markets.
• Develops and reviews all policies and procedures affecting nurse coordinator duties. Follows regulatory requirements, adheres to
National Patient Safety goals and complies with facility policy, protocols and procedures.
• Performs other duties as assigned.
Qualifications
Knowledge, Skills and Abilities
· Knowledge of fundamentals and practices of assigned nursing specialty
· Knowledge of educational requirements and instructional/teaching methods within related field
· Standards and procedures for providing specific care
· Operations, services and resources provided in the program
· Current trends and issues in healthcare, nursing practice and applicable regulatory standards
· Knowledge and use of common personal computer operating systems and databases
· Presentation and training delivery skills for teaching, mentoring and instructing individuals or groups
· Human relations and communication skills to facilitate meetings, plan, organize and implement assigned tasks
· Competent level of writing for documenting medical and educational information and preparing reports, instructions and
correspondence
· Competent level of public speaking and presentation skills to represent program to key constituent audiences
· Work collaboratively with CMC management, staff and physicians
· Strong interpersonal skills required for interviewing and compiling appropriate information
Education
· Graduate of approved nursing program required
· Bachelor's Degree in Nursing required
· CCRN or ANCC-CVN certification may be substituted for Bachelor's degree requirement
Experience
· 3 years of experience in Cardiovascular Nursing, Cardiac Surgical Nursing, Critical Care Nursing and/or experience as a
Heart Clinic Coordinator required
Licenses and Certifications
· RN - Current State of California Registered Nurse license required
· BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
· ACLS - Advanced Cardiac Life Support Certification required
· One of the following if substituting for Bachelor's degree is required
o CCRN - Certified Critical Care Registered Nurse
o ANCC-CVN - ANCC-Cardiac Vascular Nursing
