Lead Care Manager (Healthcare)



Position Summary:
The Lead Care Manager is a part of a multidisciplinary team and is responsible for coordination of all aspects of Enhanced Care Management (ECM). The Lead Care Manager oversees the provision of ECM services and implementation of the care plan, conducts outreach to assigned ECM members, engages them in enrolling into ECM, and delivers ECM services. Enhanced Care Management services are provided where the member lives, seeks care, in the office, and in the community.

Qualifications:

Bachelor of Arts or Bachelor of Science degree in Counseling, Social Work, or related field preferred or Associate of Science/Medical Assistant Certificate or Degree. Degree may be waived with equivalent work experience.
Knowledge of community resources preferred.
Proficient in Microsoft Office Suite, Outlook, Excel, Word.
Bilingual preferred.
Must be 21 years old or over.
A valid California driver's license with less than two minor moving violations.

Responsibilities:

Provide training and guidance/mentorship of frontline staff.
Implement department/program objectives.
Collaborate with administration to develop a culture of "people first".
Responsible for executing organizational strategies and initiatives.
Engage and enroll eligible members in ECM.
Oversees provision of ECM services and implementation of the care plan.
Utilize motivational interviewing, trauma-informed care and harm-reduction strategies.
Responsible for coordination of services to ensure a seamless experience and unduplicated services.
Connect members to other social services and support including transportation.
Provide Community Outreach and attend Community Events.
Provide health promotion and self-management training.
Monitor treatment adherence.
Coordinate with hospital staff on discharge plans.
Responsible for preventing and controlling infection.
Responsible for maintaining a culture of quality and safety.
Effectively use de-escalation skills and techniques for crisis prevention.
Properly administer the C-SSRS to help prevent suicide

Other:

Attend all required meetings and trainings.
Report any suspected child or dependent adult/elder abuse or neglect immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
Report any client imminent danger to self or to others or gravely disabled immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
Other duties as assigned.
