Skilled Nursing Facility (SNF) Nurse Case Manager – 100%FTE, Day - 20 S. Park (Healthcare)

Make a difference in transitions of care as an RN Skilled Nursing Facility Case Manager.

We are seeking a Skilled Nursing Facility (SNF) Nurse Case Manager to help coordinate care for individuals across the continuum.  The nurse in this role will:

  • Help to manage transitions and care efficiencies for patients residing in a SNF either short-term rehabilitation or long-term care.
  • Focus on patient experience, readmission prevention, length of stay management and transition planning.
  • Regularly meet and communicate with patients, families, SNF Case Managers, SNF APPs, social workers, physicians and other caregivers to develop transition plans of care.

Education:

Minimum – Bachelor of Science in Nursing or higher.

Preferred – Master's Degree in Nursing.

Work Experience:

Minimum – Two (2) years experience as registered nurse.

Preferred – Previous experience in post-acute care setting. Previous case management experience. Experience and proficiency using Epic.

Licenses and Certifications:

Minimum – Registration as a registered nurse in state where employed or licensure in the state licensure compact. CPR/BLS.

Preferred – Case Management Certification.

Required Skills, Knowledge, and Abilities:

  • Excellent communication, problem-solving, conflict resolution and motivational interviewing skills.
  • Excellent prioritization and organizational skills.
  • Ability to work as team member and as independent service provider.
  • Excellent verbal and written communication skills, including grammar and spelling.
  • Knowledge and experience documenting in an electronic medical record.
  • Ability to educate staff.
  • Ability to provide counseling and emotional support with compassion and respect for the individual.
  • Ability to provide support to members of the care team.
  • Ability to maintain confidentiality.

Physical Requirements:

Light: Ability to lift up to 20 pounds maximum with frequent lifting and/or carrying of objects weighing up to 10 pounds.

Work Schedule: 100%FTE, 40 hours per week. This is a Day shift position and includes eight (8) hour shifts, Monday - Friday; 8AM - 4:30PM. No weekends and no holiday rotation required.  Hours may vary based on operational needs of department.
