Manager, Clinical Case Management (hybrid) (Finance)

Supervises plans, organizes and directs the provision of out-patient population health management services, and supervises personnel and manages all functions related to utilization review, care coordination, and case management.
Position Key Accountabilities:
1. Assists with ongoing development of the UTP chronic disease management program in accordance with evidence-based guidelines and national quality initiatives.
2. Responsible for coordinating an integrated population health management program, which encompasses utilization review, case management, care coordination, population management, and social services.
3. Provides population health management and quality improvement services by regularly reviewing patient utilization data to assess current state of care, identifying gaps, and working with care teams to close identified gaps.
4. Provides backup coverage in the absence of a Clinical Case Manager.
5. Establishes policies, procedures, standards and departmental objectives consistent with UTP's strategic plans, economic, legislative regulations and regulatory requirements.
6. Adheres to the broad scope of practice as established by the Nurse Practice Act, and ensures compliance with quality standards, and regulatory and accrediting agency specifications to meet federal, state and local standards.
7. Manages human resources activities for direct reports in regards to: recruitment and selection, hiring and termination, training, professional development, mentoring, counseling, and performance evaluations.
8. Performs other duties as assigned.
Certification/Skills:
Current nursing licensure in the State of Texas, current certification in case management, and basic computing/analytic skills.
Minimum Education:
Bachelor's degree in nursing from an accredited school of nursing; master's level degree in a healthcare-related field preferred.
Minimum Experience:
Ten years of nursing experience. The 10 years of experience must include at least one year of supervisory experience, and at least five years in clinical case management, with at least one of those years in an outpatient setting.
Physical Requirements:
Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.

Residency Requirement:
Employees must permanently reside and work in the State of Texas.
