Position Summary:
The Clinical Pharmacist Specialist functions as a drug therapy expert with a comprehensive scope of practice that includes monitoring drug therapy, ordering laboratory tests, and making therapy recommendations to physicians.

Position Key Accountabilities:
1. Evaluates a variety of patients and makes appropriate drug therapy recommendations.
2. Participates in treatment team rounds on designated nursing units to implement a pharmaceutical care plan.
3. Provides consultation to other health care providers on medication therapy
4. Assists in treatment planning that ensure continuity of care for patients being discharged
5. Works with patients who have been identified as having compliance/adherence issues with their medications.
6. Reviews medication orders for appropriateness, drug selection, dosage and route of administration prior to dispensing. Performs these functions when required by staffing deficiencies.
7. Reviews patient's allergy history, drug-drug or drug-nutrient interactions for clinical relevance
8. Provides patients and family members with education on medications and lifestyle to achieve outcome goals.
9. Serves as primary clinical preceptor for TSU and UH pharmacy students.
10. Participates in research activities involving drug therapy or outcome measures
11. Oversee technicians in accordance with department policies and procedures. Ensures all patient care provided meets or exceeds all regulations, statutes, and JCAHO standards. Other duties

Certification/Skills:
Registered pharmacist in the State of Texas. Board Certification in Psychiatric Pharmacy desired but not required. **(Post pharmacist licensure) with at least 50% of time spent in Pharmacotherapy activities as defined by the Pharmacotherapy Content Outline OR Completion of a PGY-1 residency & Achieving a passing score of the Pharmacotherapy Specialty Certification Examination.
Minimum Education:
Bachelor's degree or Pharm.D. required. Residency preferred but not required.
Minimum Experience:
Completion of three (3) years of practice experience.
Physical Requirements:
Exerts up to 20 pounds of force occasionally and/or up to 10 pounds frequently and/or a negligible amount constantly to move objects.
