Fall River Health Services is looking for a Physician Assistant/Nurse Practitioner.In this role you will provide patient care services under the supervision/ collaboration of a licensed doctor of medicine who is responsible for your performance. You willdeliver care based on academic and clinical training under the supervision/collaboration of the assigned supervising/collaborating physician. A high level of expertise is required in performing and interpreting patients history, physical assessment and diagnostic tests.
Responsibilities:
Responsible for providing quality primary clinical care in consultation with the supervising/collaborating physician. This includes working with clinic staff and other medical staff members to ensure clinical standards are maintained. Assesses the health status of individuals of all ages by obtaining a data base which includes a history, physical examination, investigative studies, psychosocial assessment and development of a problem-oriented record. Makes referrals to the hospital as appropriate.Provides counseling, education and intervention for promotion, maintenance and restoration of optimal levels of health for disease prevention for individuals of all ages, families and communities.Provides assessment and appropriate management of patients of all ages with common, acute undiagnosed problems in a manner consistent with protocols for each problem which indicate the appropriate data base as well as specific assessment and management plans for a variety of selected findings. The incumbent makes referrals as appropriate.Responsible for timely completion and adequate, complete documentation of patient records and is responsible for maintaining and respecting the confidentiality of patient information in accordance with medical bylaws, HIPAA, and PA/CNP rules and regulations. This position in collaboration with facility staff, cooperates in all processes necessary to obtain timely payments.Assesses and provides follow-up management of patients of all ages with common chronic health issues in a manner consistent with protocols for each condition as well as specific impressions and management plans for a variety of selected findings.Develops professional role identity as a primary care provides, recognizing professional limitations. Refers when needed, consults and maintains a close working relationship with supervising/collaborating physician and other members of the health care team.Responsible for maintaining positive relations with patients, all health care providers, co-workers, and members of the community. This position is responsible for fostering and developing good community relations as well as direct practice development through inservice training, public health care education and participation in community health services.
