Restorative Certified Nursing Assistant (CNA) (Education)



Provide resident exercise program, direct care, and assistance in activities of daily living to residents under the supervision of a licensed nurse in accordance with facility policies and procedures.
ESSENTIAL JOB FUNCTIONS

Perform resident exercises as ordered by the physician.
Document time spent with resident and their response to exercise sessions.
Report changes in ability to responsible licensed nurse.
Obtain and record the height and weight of residents on admission and annually.
Complete and record resident weights daily, weekly, monthly, and annually.
Obtains and records monthly vital signs.
Assist/accompany residents to visits outside of the facility as needed.
Assist residents with transfers and positioning with and without use of mechanical lift or gait belt.
Turn, reposition and transfer residents with impaired mobility, alone or with assistance of others.
Assist residents with basic activities of daily living, such as, grooming, dressing, bathing, shampooing hair, shaving (both male and female) and oral hygiene and denture care.
Provide assistance with toileting routines and perineal care as indicated in resident plan of care, ensures residents are clean and dry.
Collect urine and stool specimens as needed.
Take vital signs of residents, such as temperature, blood pressure, pulse and respiratory rate and record and report information.
Measuring intake and output.
Assist residents with meals, including transportation to and from meals and/or feeding residents unable to feed themselves.
Transfer of residents from bed, chair and wheelchair, etc.
Assist residents with the use of walkers, wheelchair, canes and braces.
Positioning of residents in chairs, beds and gerichairs.
Turning and changing positions of residents
Lift residents both manually and with a mechanical lift.
Assist residents with performing range of motion exercises (active and passive).
Assist residents with ambulation.
Assist in light housekeeping duties, such as, making occupied and unoccupied beds and ensuring that resident rooms and common areas are free of trip risks or other safety hazards.
Keep equipment, supplies and resident furniture clean.
Weigh the residents, record and report information.
Provide post mortem care.
Practice universal precaution and follow isolation techniques.
Participate in the development and implementation of resident care plans.
Review resident care plans and perform care consistent with guidelines and directives as contained in the Care Plan.
Assist in assuring the maximum quality of life for all residents by maintaining privacy, accommodating individual preference, promoting independence, encouraging and assisting participation in facility activities and events, and communicate respectfully at all times.
Conduct resident rounds as assigned to ensure residents are neat, clean, safe and comfortable.
Direct resident and visitor complaints to the Charge Nurse or Unit Nurse Supervisor.
Respond to all call lights promptly in facilities with call lights. Keep call light within reach of non-ambulatory resident when unattended.
Perform blood glucose measurements after completion of training with licensed nurse.
Maintain continuous communication with team leader. Report changes in resident condition to licensed nurse, including but not limited to changes in food or liquid intake, toileting, vital signs or behavior.
Assist physicians and nurses with treatments and procedures.
Attend staff development activites to improve knowledge and skills and to maintain competency and certification.
Complete all showers before the end of shift.
Sign all doors.
Chart BM's.
Document all care provided in resident record/C.N.A. notes in a timely manner following facility procedures.
Assist new employees in acclimating to facility policies, procedures and routines.
Demonstrates genuine care for residents through compassion, kindness and gentleness.
Assist nurses and admission staff in following facility procedures for admission, transfer and discharge.
Scan out all central supply items used for each resident.
Prepare rooms for new admissions.
Mark resident's personal belongings and take inventory of personal possessions.
Transport and escort residents as necessary
Consistently work cooperatively and effectively communicate with residents, charge nurses, other C.N.A.'s, other departments and outside caregivers, families and vendors.

KNOWLEDGE, SKILLS AND ABILITIES
Knowledge of:

Medical terminology.
Safety rules, policies and procedures, infection prevention and control procedures.
Facility emergency/disaster response and evacuation procedures.
Resident Rights and identifying abuse and/or neglect.

Skill in:

Use and care of equipment, tools and materials used in resident care activities.
Working independently to complete daily activities according to work schedule.

Communication both oral and written.
Following and maintaining safety standards.
Following and effectively communicating verbal and written instructions.
Operating a computer utilizing a variety of commonly used and specialized software applications.

Ability to:

To use diagnostic equipment.
Establish and maintain effective working relationships with associate personnel, residents and resident family members.
Read and write clearly.
Understand and follow, quickly and accurately, oral and written instructions.
Prioritize, including excellent time management skills.
Maintain confidentiality and work under pressure in a dynamic environment with changing demands and priorities.
Comply with policies and procedures.

WORKING CONDITIONS/PHYSICAL REQUIREMENTS

Must be able to bear weight of 100 pounds or more.
Requires constant physical activities including standing, walking, lifting, bending, stooping, pulling, stretching, turning, grasping and reaching.
Must be able to wear protective equipment such as gloves, masks, gowns, goggles and gait belts.
Visual acuity to determine the accuracy, thoroughness and quality of work assigned.
Aural acuity to receive detailed information through oral communication, often from persons with speech impairment.
Safety-sensitive position pursuant to A.R.S. 23-493.

REQUIRED MINIMUM EDUCATION, EXPERIENCE AND TRAINING

High school diploma or its equivalent.
Must hold a current state certified nursing assistant license and must maintain license renewal in accordance with the standards of the State Board of Nursing.
Minimum experience of one year preferred.
Previous geriatric care preferred

SPECIAL JOB REQUIREMENT(S)

Obtain/maintain Arizona Department of Public Safety Fingerprint Card at employee's expense.
Obtain/maintain CPR Certification.
Obtain COVID-19 vaccine or an approved exemption
Complete annual Tuberculosis Testing.
Complete required 12 hours of annual in-service training.
Mandated federal, state, and local communicable disease testing as required.

All employees of the facility are subject to immediate callout when an emergency or disaster occurs. All employees are considered to be available for these types of events and will immediately respond, in accordance with the facility's Emergency Operation Plan, when directed by the Administrator, or when directed by their department director and/or immediate supervisor. Such call to respond and assist may involve those activities assigned to their regular job responsibilities and incorporate other non-job related duties as necessary and required of an employee to respond to the emergent matter at hand. An employee's response to these matters may require the working of unusual, long hours over a possibility of an extended period of time with infrequent breaks or rest periods. An employee who is working with an accommodation must meet with their supervisor and department director to discuss their requirement for response.
