School Based Long Term Services & Supports Care Coordinator (Hybrid, Morrisville, NC Based) (R&D)
The School Based Long Term Services & Supports Care Coordinator provides comprehensive care coordination and monitoring in collaboration with Wake County Public School System and Special Education Department to provide guidance and planning for students who are identified as needing support with the I/DD eligibility process, Innovations consumers, and consumer's transitioning to or from placement at crisis facilities.
This position will allow the successful candidate to work a hybrid schedule, both remote and onsite. Candidates are required to travel to Wake County; a minimum of two days per week.
Responsibilities & Duties
Provide Care Coordination
Collaborate and share information with internal and external stakeholders
Develop relationships with community partners to support the population
Minimum Requirements
Education & Experience
Bachelor's degree from an accredited college or university in a human service field and two (2) years of full-time, post-bachelor's degree accumulated I/DD experience with the population served
OR
Bachelor's degree from an accredited college or university in a non-human service field and four (4) years of full-time, post-bachelor's degree accumulated I/DD experience with the population served

Salary Range
$22.16-$38.15/hourly
Exact compensation will be determined based on the candidate's education, experience, external market data and consideration of internal equity.
