Family Group Decision Making Consultant (Personal Services)

QUALIFICATIONS

  1. Bachelor's Degree required within one year of start date, with major course work in sociology, social welfare, psychology, gerontology, anthropology, criminal justice, criminology, theology, nursing, counseling, education or other related social science or human services field.
  2. One to three years working with families in a mental health setting is required.
  3. One to three years case management experience required.

REQUIREMENTS

  1. Must possess excellent verbal and written communication skills.
  2. Must have a valid driver's license, acceptable driving record, and access to a reliable automobile.
  3. Ability to adapt hours to needs of position.  May need to work evenings and/or weekends hours periodically including providing on-call emergency services on a rotating basis.
  4. Access to a telephone.
  5. Ability to remain organized despite pressured situations.
  6. Must be eligible for PA State Police clearance, PA Child Abuse History Clearance, FBI Clearance, Arrest/Conviction Report and Certification Form, and Act 168 Forms. 
  7. Must be able to identify with the agency's Mission Statement and support the agency's Core Values.
  8. Be able to work independently and as part of a team. 
  9. Maintain confidentiality and adhere to ethical standards
CORE COMPETENCIES:
1.  Facilitation:
  • Guide families through the FGDM process, ensuring all participants feel heard and valued. 
  • Help families identify their strengths and concerns, and develop culturally sensitive plans. 
  • Work with families to identify individuals to participate in the FGDM process
  • Facilitate the FGDM meeting, ensuring a safe and supportive environment for all participants.
2. Preparation and Coordination:
  • Prepare families and participants for the FGDM conference, explaining the process and purpose. 
  • Collaborate with families to identify and invite relevant family members, friends, and service providers. 
  • Ensure all participants are informed and prepared for the conference. 
3.  Support and Advocacy:
  • Provide ongoing support to families as they implement their plans. 
  • Advocate for families' needs and ensure their voices are heard. 
  • Connect families with resources and services that can support their goals. 
4.  Assessment and Planning:
  • Assist families in identifying and addressing concerns related to child safety, well-being, and permanency. 
  • Help families develop and implement plans that are culturally sensitive and meet their specific needs. 
5.  Monitor:
  • Collect and tracking data
  • Analyzing outcomes
  • Reporting back to GR and CYS etc.
  • Evaluate the effectiveness of the family plan of the course of 30-60 days
6.  Collaboration:
  • Work collaboratively with other professionals, such as caseworkers, therapists, and service providers. 
  • Maintain open communication with all stakeholders involved in the FGDM process.
  • Support Family Empowerment Program 
  • Support with families case management needs
  • Support with program implementation and development
  • Support with On-Call rotation
