Occupational Therapist (Healthcare)



Occupational Therapist - Lime Ridge Healthcare LLC dba Concord Post Acute - Concord, CA.

Job Duties: Provide evidence-based occupational therapy services, follow applicable federal, state, and local regulations as well as internal policies and procedures, and strive to uphold the values of the organization in all professional interactions. Provide comprehensive evaluation of assigned/scheduled patients including all pertinent systems to identify impairments and functional deficits within scope of practice. Create a Plan of Care designed to achieve appropriate person-centered outcomes. Provide treatments according to plan of care and/or delegate to appropriate personnel. Utilize internal clinical programs and pathways as outlined to ensure evidence-based practice and optimal patient outcomes. Monitor and update Plan of Care to accommodate for progress, barriers, and/or change of conditions or circumstances. Coordinate and communicate with all members of the interdisciplinary team to ensure successful outcomes. Complete screens, wellness interventions, and/or personal fitness training services as needed and requested according to internal policies and procedures. Complete documentation of services according to all applicable internal documentation policies and procedures. Record and bill services according to payer regulations and internal billing policies and procedures. Record labor time according to Federal, State, and local regulations and internal payroll policies and procedures. Comply with all federal, state, and local regulations and internal policies and procedures including but not limited to those related to fraud, waste, abuse, resident rights, CMS rules of participation, HIPAA, dress code, and safety. Attend and participate appropriately in all mandatory trainings. Take personal responsibility for completing action plans related to improving and ensuring compliance. Demonstrate practice of internal compliance principles including advocacy, resource stewardship, and integrity. Attend and participate in facility meetings, programs, and trainings. Mentor and develop new employees, students, and therapist assistants. Complete travel opportunities as presented and accepted. Participate in Vital Life Foundation Programs and New Chapter Events. Respect all people, treating others with dignity, kindness, and professionalism. Be a champion of the journey by maximizing function in physical, emotional, social, and cognitive realms for patients, families, co-workers, and self. Pursue opportunities to learn and grow, mentor others, and move the organization to new levels of excellence. Demonstrate organizational capacity including time/energy management sufficient to meet the demands of the job. Effectively and professionally communicate in written, verbal, non-verbal, and small group presentation modes. Inspire others by creating engaging, positive, and challenging experiences while providing honest and appropriate developmental feedback. Make informed decisions in a timely manner to benefit stakeholders. Achieve targeted results in business, quality, and customer satisfaction. Utilize Microsoft Outlook to view and respond to emails. Efficiently use internal electronic documentation and billing system.

Minimum Requirements: Master's degree in Occupational Therapy. Valid NBCOT certification. Valid State of California Occupational Therapist license.

Must have legal authority to work in the U.S. EEOE.
