Primary Purpose: Residential: The Clinical Coordinator will conduct and prepare psychosocial assessments, offer therapeutic interventions, and lead the discharge planning process for patients and families receiving residential services at the Lindner Center of HOPE. The Clinical Coordinator is responsible for serving as the champion for the patient experience and is the clinical communication liaison between the patient, family, referrers and other stakeholders. The Clinical Coordinator is the key resource for identification, facilitation, planning and logistics for next level discharge resources that may be needed and ensuring that post hospitalization needs are met.Required Skills and Knowledge:
Education: Master of Social Work degree from an accredited school of social work.
License & Certification: Licensed Social Worker or Licensed Independent Social Worker by the State of Ohio Social Work Board.
Years Experience: 1-3 years experience in a health care setting. Psychiatric/mental health experience required.
Required Skills and Knowledge: Solid knowledge of socialwork practice; strong assessment, interview and counseling skills. Strong interpersonal skills; ability to collaborate well. Ability to enthusiastically follow and model the Lindner Center of HOPE mission, vision and values.
