WHAT YOU WILL DO:
Serve as the primary link between healthcare providers and our care services.Build and maintain strong relationships with case managers, discharge planners, resident care directors, social workers, physicians, hospitals, and other referral sources you may have.Assess potential patients' clinical needs and communicate those effectively to our care team.Educate referrers and their patients about our services and coordinate smooth transitions to our care team.
