Behavioral Health Care Coordinator (Finance)
Mary Ryder Home, a Provident Behavioral Health residential program is adding a Behavioral Health Care Coordinator to help support our mission. As the Behavioral Health Care Coordinator, you will work with a team of providers to develop and enhance residents mental wellbeing, stability, self-sufficiency, and skills to live more independently. As Mary Ryder Home is licensed as an Intensive Residential Treatment Setting (IRTS), this role is integral in helping our residents to learn how to manage their behavioral health through therapy, development of coping skills, and learning to care for themselves.
The schedule is part-time (20 hours per week) M-F with some evenings required.
What youll do:
Participate or conduct daily programming, with the support of the Clinical Supervisor, to enhance the wellbeing and independent living skills of the residents in the program Document any changes in residents behavior, mood, eating habits, or medical concerns in our Electronic Health Record System (EHR) Dispense or administer medications as ordered by the physician and document in our EHR system Use strength-based intervention to enhance a residents coping skills Teach residents self-care skills such as proper hygiene Help maintain the cleanliness and safety of the facility by active participation and modeling Teaching residents the importance of physical wellness by conducting fitness activities Participate in regular supervision with the Clinical Supervisor Assist the Clinical Supervisor in resident goal implementation Participate in quality improvement meetings and program evaluation Participate in resident treatment planning Engage residents in one-on-one coaching and skill building Practice HIPAA and Confidentiality Standards Attend trainings, as assigned by the Clinical Supervisor Provide resident oversite and support
What we need you to have:
Bachelors Degree in Counseling, Social Work, Psychology, or any other health-related field Current and ongoing Level One Medication Aid certificate (Insulin Certified a plus). Training is provided upon hire, with L1MA certification required before the first shift working with residents. Current and ongoing CPR/AED/First Aid training provided upon hire Experience in a long-term care facility a plus Passion for working with older adults
