Eligibility Specialist (Personal Services)
Job Summary:
Under the supervision of the Health Plan & Housing Manager, facilitates the process of securing documentation for PACE participants to support health plan eligibility and supportive housing eligibility for all PACE facilities and programs. Will engage directly with the participant, in person and by phone and will accompany him/her to appointments as needed, to advocate and help represent him/her at benefits offices, DMV, and/or other community resources supporting procuring of required documents for either PACE eligibility or supportive housing applications. Will provide participant and family education, benefits counseling and available, applicable resources. The Eligibility Specialist will receive all assigned cases through the Health Plan and Housing Manager. As assigned, the Eligibility Specialist will connect with the PACE Social Work department case managers to receive individualized case information, recommended approaches, care plan clarification, and specific eligibility and application needs.
Education and Experience:
Bachelor's Degree from an accredited university in Social Work or a degree in Gerontology or other related Health and Human Services fields. A minimum of one year experience working with a frail or elderly population required. Experience working on a multidisciplinary team in a hospital, nursing home or community-based setting preferable. A minimum of one year of experience or related training required.
